Exception Reporting

If you are in a training program then exception reports should be submitted through the trust's approved system or via chelwest.guardianofsafeworking@nhs.net

Fellows

If you are in ICU fellow post then you can submit exception reports on CLWrota. While 'exception reports' are only part of the newer contract (which at the time of writing, the ICU fellows are not on). To submit a report on CLW:

1. Open the CLWrota app

2. Navigate to the 'My Rota' section (this is the default view when the app loads)

3. Tap on the shift you have worked extra hours

4. Scroll to the 'Make an annotation' section at the bottom

5. Select 'Exception Report (public)' as the type

6. Enter the details of the uncontracted hours you have worked

Exception reporting helps ensure we are funded for the right number of staff so please submit a report if applicable.
